Getting Started by Particular Plan

Step 1: Register on kp.org

To register on kp.org: click here and select 'register' in the upper right hand corner of your screen.

Step 2: Select your plan below

Select your plan below to review general plan benefits (find your plan name in documents provided by your Employer). 

For 'Forms & Documents' specific to your plan (e.g., claims forms, benefit details, etc.), please scroll to the bottom of your plan section.

Note: For KP members switching to a Level Funded arrangement, review your plan benefits as some may differ from your previous KP plan (e.g., formulary, provider options, etc.).

Your Plan provides exclusive access to our top-notch Kaiser Permanente Network Providers and hospitals. We work together to create a personalized wellness plan that makes it easier for you to stay healthy — in mind and body.

To manage your prescriptions, learn more about our pharmacy services, or locate the specific pharmacies available to you, log on to kp.org and select pharmacy.

Care While Traveling:

Members who are traveling away from home, can plan ahead by visiting our Health Care Away from Home site. 24/7 care is available from Kaiser Permanente health care professionals by phone or video.

Exclusive Provider Organization Plus (EPO Plus)

 

Provider and Care Options:

Your Plan provides exclusive access to care from Kaiser Permanente Network Providers and Non-Participating Providers (for a select number of services, see your plan details for more information). We work together to create a personalized wellness plan that makes it easier for you to stay healthy — in mind and body.

To make an appointment please select a provider according to the appropriate Tier. For questions regarding your plan or Tier type, please contact Customer Service using the phone number listed on the back of your Physical ID Card, Digital ID Card, or Plan Documents:

  • In-Network Services: To find a Kaiser Permanente Provider, login in to kp.org.
  • Out-of-Network Services: Your plan includes a select number of services/visits* with any other licensed provider. For qualified services and details, refer to your specific plan in the 'Forms and Documents' section below (your plan name can be found within your Plan Materials). 
    • Key services/items that count as services/visits:
      • Routine office visits, including primary, preventive and specialty care, mental health and chemical dependency visits
      • Diagnostic lab tests, diagnostic X-rays, and selected articles of durable medical equipment count separately toward visit limit
      • Physical, occupational, and speech therapy office visits
      • Allergy injections received at an office

*Services/Visits - one visit may include multiple services. For Example:  If a member sees an Out-of-Network Provider and the provider orders a lab test done in his/her office, this will count as two Plus visits.

Please note cost shares vary based upon your provider selection and will apply to preventative health services.

Pharmacy Options:

  • In-Network: Members have access to Kaiser Permanente Pharmacies. To locate a KP pharmacy, log into kp.org and select Pharmacy.
  • Out-of-Network: Members have access to any other licensed pharmacy. If an Rx script is not filled through a Plan Pharmacy, the Rx script will go toward the member's Plus Benefit.

Please note that not all Plus Plans have Out-of-Network Pharmacy options. Refer to your plan documents for details.

Please note: Members may fill a prescription written from an out-of-network provider at a KP pharmacy for cost savings. Cost shares and fill quantity may differ per tier/option, check plan for details.

Provider Care and Options:
  • Participating Provider Tier 1: You may select a provider from First Health Network Participating Providers (for service in KP states: CA, CO, GA, HI, MD, OR, VA, WA, and DC) or the CignaPPONetwork* (for service in non-KP states).
  • Non-Participating Provider Tier 2: Any other licensed provider.

Colorado Level Funded PPO members can fill a prescription written by any licensed provider at a KP Pharmacy, or any licensed pharmacy (please note cost shares differ per selection). 

  • Pharmacy options include:
    • Participating Pharmacy Tier 1:  OptumRx pharmacies. Register on OptumRx.com (using Kaiser Permanente Member Record Number to locate participating pharmacies).
    • Non-Participating Pharmacy Tier 2: Any other pharmacy. Note: you may need to pay out of pocket, then be reimbursed using the OptumRx Pharmacy Claim Form (see “Documents and Forms” below).

Members may fill a prescription written from any provider, at any pharmacy option listed. Cost shares may vary per tier, check plan for details.
